Policies & Procedures
Thank you for choosing Wellness RN PLLC for your hydration and wellness needs. Please carefully review the following service agreement outlining our policies and terms of treatment for both mobile services and our IV Suite.
Good Faith Assessment Requirement
Before any treatment is rendered, all clients must complete a Good Faith Assessment (GFA) with our provider to determine eligibility for IV therapy. This ensures that treatment is appropriate and safe based on your medical history and current health status.
Right to Refuse Treatment
Wellness RN PLLC reserves the right to refuse treatment at our sole discretion. This includes, but is not limited to, clients who:

Are deemed medically inappropriate for treatment

Exhibit signs of intoxication or impairment

Display disruptive or inappropriate behavior

Fail to complete the required Good Faith Assessment
Mobile IV Therapy Policies
For clients receiving IV therapy at their preferred location:
A safe, clean, and accessible environment must be provided for treatment
Clients must be present at the scheduled appointment time
Cancellations or rescheduling requests must be made at least 48 hours in advance to avoid cancellation fees
If the location is deemed unsuitable for treatment upon arrival, the appointment may be canceled at the nurse’s discretion
IV Suite Policies
For clients receiving IV therapy at our IV Suite in Milton, NY:
Appointments are required; walk-ins are not guaranteed service
Clients must arrive on time. A 15-minute grace period is provided, after which the appointment may be rescheduled or canceled
Cancellations or rescheduling must be made at least 48 hours in advance
Guests accompanying clients are limited to one person per appointment
Administration & Safety
IV treatments are prepared and administered by Registered Nurses (RNs) following strict aseptic techniques to ensure safety and minimize infection risk
Clients must disclose any known allergies, medical conditions, or medications prior to treatment
While IV therapy is generally safe, clients acknowledge the potential risks, including bruising, irritation at the injection site, or other mild side effects
Acknowledgment & Consent
By receiving services from Wellness RN PLLC, you acknowledge and agree to the policies outlined in this service agreement. You understand the necessity of the Good Faith Assessment, the possibility of treatment refusal based on provider discretion, and the importance of adhering to both mobile and IV Suite policies.

Cancellation Policy

Appointments must be canceled or rescheduled at least 48 hours in advance. A 15-minute grace period is allowed for lateness; however, please contact us to inform us if you are running late. If we do not receive prior notice, we reserve the right to cancel the appointment after the grace period.
A 72-hour email reminder will be sent before your appointment to allow time for confirmation or necessary changes.
Cancellations made less than 48 hours before the appointment will result in the forfeiture of your payment.​
