Good Faith Exam (GFE)
REQUIRED BEFORE IV Hydration and Vitamin Treatment
Service Description
This is a required call with our licensed provider who will review the information from your completed medical questionnaire to ensure IV hydration and vitamin therapy are safe and appropriate for you. Important: Please complete your medical questionnaire prior to your appointment so our provider can review it during the call. Once completed, your Good Faith Exam is valid for 1 year, and you won’t need to repeat it unless your health status changes.
Cancellation Policy
Cancellation & GFE Policy A deposit is required to book all IV drip and vitamin shot appointments. 48-Hour Notice Required → Cancellations or reschedules must be made at least 48 hours in advance. Late Cancellations → Deposits are forfeited if you cancel or reschedule with less than 48 hours’ notice. No-Shows → Clients who fail to cancel or attend their appointment will be charged or invoiced for the full service amount. Good Faith Exam (GFE) Required → A virtual consultation with our Nurse Practitioner is required once every 12 months to ensure IV hydration and vitamin therapy are safe and appropriate.
